Job summary

University of Alabama at Birmingham (UAB) is seeking a Part-Time RN-Clinical Care Coordinator to coordinate multidisciplinary clinic activities, manage patient referrals, and ensure timely access to services. The role requires RN licensure, BLS, and several years of clinical nursing experience with a focus on care coordination.

Responsibilities include reviewing records, scheduling, patient navigation, and collaboration with social work and nutrition services.

Qualifications

• Bachelor's degree in Nursing plus three years of relevant clinical nursing experience OR Associate's degree in Nursing plus five years of relevant clinical nursing experience.
• Currently licensed or eligible to be licensed as an RN by the Alabama Board of Nursing.
• BLS certification from the American Heart Association or to be obtained within 30 days of employment.
• Depending on unit/department, ACLS and/or PALS may be required.

Responsibilities

• Obtains and reviews medical records for new and established patients and ensures information is available prior to visits.
• Coordinates the planning and organization of multidisciplinary clinic activities and schedules.
• Serves as a primary contact for patients and families, assisting with referrals, scheduling, and navigation of services.
• Builds relationships with patients and families to support continuity of care.
• Monitors referrals and social work needs and connects patients with appropriate resources.
• Coordinates with the UAB social worker and community resources.
• Manages daily clinical work pool and supports workflow standardization across EMR systems.
• Reviews charts to coordinate appointments and reduce unnecessary testing.
• Performs other duties as assigned.
