Job summary

University of Alabama at Birmingham seeks an RN-Clinical Care Coordinator to manage a patient caseload, coordinate with the multidisciplinary team, and plan care with physicians and staff to achieve timely, quality outcomes. The role may include coordinating follow-up clinics and scheduling tests or surgeries.

The position requires RN licensure and a strong focus on care pathways, patient advocacy, and collaborative rounds with the surgical and GI teams. This is an on-site role with day shifts.

Qualifications

  • Bachelor's degree in Nursing and three (3) years of relevant clinical nursing experience.
  • OR Associate's degree in Nursing and five (5) years of relevant clinical nursing experience required.
  • Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing required.

RN-CLINICAL CARE COORDINATOR

General Responsibilities

To manage the plan of care for a caseload of patients, assuming 24-hour responsibility for patient caseload.

To serve as the liaison with the multidisciplinary care team.

To plan care management as achieved through coordination of care with physicians, nursing staff and other health professionals to ensure high quality patient outcomes are accomplished within the established time frame and appropriate use of resources.

May coordinate patient follow-up clinics and schedule tests and surgeries.

Key Duties &Responsibilities

Facilitates implementation of plan of care for designated patient caseload.

Participates in on-going staff development.

Promotes quality, cost-effective management of patient care.

Develops, initiates and monitors patient care pathways that identify an expected length of stay, goals of admission and discharge plan.

Assumes responsibility for personal professional growth and development.

Performs other duties as assigned.

Additional Duties & Responsibilities

Act as a bridge between patients, families, and surgeons

Schedule GI surgeries, endoscopies, and follow-up visits

Attend rounds and coordinate multidisciplinary care

Salary Range: $65,582.40- $86,382.40

Qualifications
Education

Bachelor's degree in Nursing and three (3) years of relevant clinical nursing experience

OR Associate's degree in Nursing and five (5) years of relevant clinical nursing experience required.

Bachelor's degree in Nursing preferred.

Work experience may NOT substitute for education requirement.

License/Certifications

Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing required.

Certified in Basic Life Support (BLS) from the American Heart Association (AHA) is required or should be obtained within 30 days of employment. Depending on the unit/department, Advanced Cardiac Life Support (ACLS) certification and/or Pediatric Advanced Life Support (PALS) from the American Heart Association (AHA) may be required.
