Job summary

Acentra Health is seeking an RN Clinical Assessor to complete needs-based eligibility determinations for Medicaid-funded PCS in Granville and Vance counties, NC. The role combines remote work with field visits to homes and assisted living facilities, with travel requirements of up to 60 miles from home base and potentially longer daily drives.

Responsibilities include conducting assessments for PCS eligibility, determining level of care to support home-based services, and guiding beneficiaries

Qualifications

• RN license for North Carolina or compact multi-state.
• Valid driver's license and reliable transportation.
• Computer proficiency in Microsoft Office Suite.

Responsibilities

• Conduct assessments to determine PCS eligibility using state-approved tools.
• Assess level of care to help beneficiaries remain in home/community.
• Hybrid role with remote work and field visits; travel up to 60 miles from home base, potentially 120+ miles in a day.
• Interview family members and informal caregivers during assessments.
• Provide guidance to select PCS providers and conduct service plan reviews.
• Submit completed assessments via state-approved interface; participate in mediation/appeals.
• Respond to state inquiries and attend staff meetings/audits.
• Perform initial eligibility determinations for 1915(c) HCBS; annual/change of status assessments.
