This full-time position supports home visits and telehealth assessments for Veterans and caregivers, emphasizing care coordination and evidence-based practice. Qualifications

• English language proficiency required for direct patient care.
• Graduate of an approved nursing program with valid state license.
• Nursing education across ADN/ diploma, BSN or MSN pathways accepted depending on licensure.
• State licensure and active registration required; may include temporary appointment until licensure obtained.

Responsabilités

• Perform clinical assessments for Veterans and caregivers under CSP.
• Conduct home visits and telehealth assessments.
• Coordinate care and referrals to VA and community supports.
• Document CSP assessments and ensure compliance with standards and guidelines.
• Participate in ongoing performance improvement and care management.
