Job summary

The VA San Diego Healthcare System in California is seeking a Caregiver Support RN Clinical Assessor to provide direct nursing care, enroll new applicants, and coordinate in-home assessments for Veterans and Caregivers.

You will work within the Caregiver Support Program, perform functional assessments, advocate for Veterans, and collaborate with multidisciplinary teams to improve access to benefits.

Qualifications

• English proficiency required for direct patient care and documentation.
• Current, full, active nursing license in the United States.

Responsibilities

• Provide direct nursing assessments and plan interventions for Veterans and Caregivers.
• Complete enrollments and intake for Veteran and Caregiver applications.
• Administer assessments via phone, telehealth, or in-clinic visits.
• Advocate for Caregiver/Veteran dyads to ensure services and benefits.
• Coordinate with Central Eligibility Team and other internal resources.
• Support education and training tools for Caregiver Program readiness.
