Job summary
AdventHealth in Sebring, FL seeks a registered nurse to join our care team. You will assess patients’ needs, develop care plans with an interdisciplinary team, and provide patient and family education.
You will coordinate care for high-risk patients, participate in group visits, and support transitions of care while maintaining accurate documentation in the electronic health record. RN licensure and BLS are required.
Qualifications
- Associate degree in nursing required; RN license required.
- Preferred Bachelor’s degree in nursing.
- Experience with patient education and care planning preferred.
Responsibilities
- Assess patients’ physical, psychosocial, cultural, and spiritual needs.
- Develop and implement patient care plans with inter-disciplinary team.
- Educate patients and families and evaluate teaching effectiveness.
- Coordinate high-risk patient group visits and transitions of care.
- Maintain accurate nursing documentation in the EHR.
