Job summary

MemorialCare in Long Beach is seeking a dedicated RN for the Outpatient Hospice team. This per-diem, day-shift role supports symptom management and family teaching to maximize quality of life.

Minimum 3 years as an acute care RN; California RN license required; BSN preferred; BLS certification. EMR experience and strong communication are essential to coordinate care with the IDT and nursing staff.

Qualifications

• Minimum 3 year’s relevant experience as a Registered Nurse in an acute care setting. Home Health, Oncology, and/or Hospice experience preferred.
• Effective problem-solving skills; good interpersonal and communication skills; ability to relate to a variety of health professionals in different settings.
• EMR experience

• Assesses present and potential needs of patient and family
• Review Plan of Care with patient, family, physician, IDT and review/evaluate/modify routinely
• Establish realistic goals of care to ensure continuity and quality of care
• Complete all documentation in a concise, timely manner per agency standards

Position Summary

This position requires the full understanding and active participation in fulfilling the mission of Saddleback Medical Center. The employee shall support Saddleback Medical Center’s strategic plan and participate in and advocate performance improvement/patient safety activities. To provide intermittent visits to the Hospice and Palliative patient and family, providing symptom management, and teaching to maximize quality of life. Coordinates the efforts of the team, including medical, nursing and psychosocial support to provide appropriate care as the patient transitions to the end of life.

• Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities. Minimum Qualifications
