Job summary

Signature HealthCARE of Spencer County is seeking an experienced RN to join our skilled nursing team. The RN will provide direct nursing care, administer medications, and supervise CNAs to ensure high-quality resident outcomes.

You will collaborate with an interdisciplinary team to develop and review individualized care plans, maintain thorough documentation, and coordinate admissions and discharges while ensuring regulatory compliance. LTC experience and EHR proficiency are preferred.

Qualifications

• Active RN license in the state of practice.
• Minimum 1 year of clinical nursing experience; long-term care experience preferred.
• Supervisory or charge nurse experience preferred.
• CPR/BLS certification preferred.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Supervise and direct nursing assistants and medication aides during the shift.
• Monitor residents' status and notify physicians of changes.
• Develop, implement, and review individualized care plans with the interdisciplinary team.
• Complete accurate clinical documentation and coordinate admissions/transfers/discharges.
• Participate in QA activities and staff education as needed.
