Job summary

Lutheran Homes Of South Carolina is seeking a Registered Nurse to provide comprehensive nursing care at Heritage at Lowman Nursing Center in Chapin, SC. The RN will assess residents, administer medications, coordinate care plans, and supervise nursing staff while upholding residents’ comfort, privacy, and dignity.

The role requires an active RN license in South Carolina, CPR certification, and prior LTC facility experience is preferred.

Qualifications

• RN license in the state of practice.
• CPR certification required.
• Experience in a long-term care or skilled nursing facility preferred.

Responsibilities

• Provide direct care to residents according to individualized care plans.
• Provide compassionate, high-quality care and exceptional customer service to residents and families.
• Assist with daily activities, including meals and hydration.
• Assess resident care situations and initiate nursing interventions.
• Observe, monitor, and document changes in resident conditions; update care plans as needed.
• Administer and document physician-ordered medications and treatments; monitor responses.
• Maintain communication with the Director of Nursing and other healthcare team members; participate in care plan meetings.
• Participate in Quality Assurance reviews; ensure HIPAA compliance.
• Plan, supervise, and evaluate nursing staff; conduct rounds and address changes in residents’ conditions.
• Maintain a safe and clean environment; report safety hazards and equipment issues.
