Summary of Job Duties: The Registered Nurse promotes wellness, improves outcomes, and supports self-care within a defined patient population. The RN III provides support, education, and assistance to patients with chronic conditions, developing and implementing individual health plans, and supporting self-management. Coordinates outpatient care, prescription refills, and patient education in collaboration with physicians. The role functions as a Charge Nurse and may float to other clinics as needed.
