Summary of Job Duties: The RN will promote wellness, improve outcomes, and support self-care within a patient population. The RN III will support patients with chronic conditions by providing education and assistance in disease management, developing and implementing health plans, and coordinating care, including prescription refills and patient education in collaboration with physicians. The role requires a commitment to patient outcomes and high performance levels despite workload variations. This position functions as a Charge Nurse and may float to other clinics as needed.
