Job summary

Community Care, Inc. is seeking a compassionate Registered Nurse (RN) to provide in-home health care to clients in the Westmoreland and Fayette County area.

The role involves skilled nursing visits, care plan development, patient education, and coordination with a multidisciplinary team in a supportive home setting. Requirements include PA RN license, 1 year of clinical nursing experience (home health preferred), CPR certification, and reliable transportation.

Qualifications

• Current Registered Nurse (RN) license in the state of Pennsylvania.
• Graduate of an accredited nursing program.
• Minimum 1 year of clinical nursing experience (Home Health experience preferred).
• CPR certification (required).
• Reliable transportation and a valid driver’s license.
• A caring heart, strong independence, and a passion for pediatric care.

Responsibilities

• Conduct skilled nursing visits in patients' homes (assessment, reassessment, medication administration, IV therapy, and treatments).
• Develop and implement individualized care plans.
• Educate patients and caregivers on managing conditions and medications.
• Collaborate with a multidisciplinary team to ensure continuity of care.
• Document visits and patient progress accurately and timely.
