Job summary

Qualifications

• 2 years experience in a clinical settings (e.g. home health, inpatient, physician office, clinic).
• BLS (Basic Life Support) from ARC or AHA within 30 days of hire.
• RN license in Oklahoma or eNLC multistate license.
• Case Management Certification required.
• Bachelor's of Science in Nursing awarded from an accredited program.

Responsibilities

• Completes comprehensive assessment of patient’s clinical, psychosocial and financial needs using available resources.
• Coordinates timely transfers to appropriate levels of care per clinical needs and utilization criteria.
• Develops and updates discharge plan with referrals to health care and community resources.
• Communicates discharge plan and changes to patient, family and health care professionals.
