Job summary

INTEGRIS Health in Oklahoma City is seeking a full-time RN Case Manager for day shift, every other weekend, to join INTEGRIS Baptist Medical Center. You will assess patient needs, coordinate care transitions, and collaborate with physicians, discharge planners, and care teams.

Required: 2 years in clinical settings, active RN license in Oklahoma or eNLC, BLS, strong communication skills; Bachelor's of Science in Nursing and Case Management Certification preferred. Valid driver’s license required.

Qualifications

• 2 years experience in a clinical settings.
• BLS certification within 30 days of hire.
• RN license in Oklahoma or eNLC.
• Excellent interpersonal communication and collaboration skills.
• Must communicate effectively in English; valid Oklahoma driver license.

Responsibilities

• Completes comprehensive assessment of patients' clinical, psychological and financial needs.
• Coordinates timely transfers to appropriate levels of care as indicated.
• Develops, implements and revises discharge plans, including referrals to care resources.
• Communicates discharge plan to patient, family and healthcare professionals.
• Assists with utilization of resources and timely discharge planning.
• Facilitates patient transfers and liaises with departments and providers.
