Job summary

Qualifications

• Education level equivalent to completion of an associate degree in nursing.
• Clinical competence in nursing preferred.
• Current Georgia nursing license; BLS Provider certification required.
• Experience with chart auditing and interpreting regulations governing patient care.

Responsibilities

• Assess treatment needs and develop, monitor, and evaluate treatment plans.
• Facilitate inter-disciplinary care activities.
• Coordinate Swing Bed Program and continued care after patient transition.
