Job summary

Paycom is seeking a Case Manager in Hinesville, GA to assess patient needs, develop and monitor treatment plans, and coordinate inter-disciplinary care. The role reports to the Chief Nursing Officer and supports the Swing Bed Program to ensure smooth transitions of care.

The candidate should hold an active Georgia nursing license and a current BLS Provider certification, with experience in chart auditing and regulatory interpretation. On-site position with hospital-affiliated responsibilities.

Qualifications

• Education level equivalent to completion of an associate degree in nursing.
• Clinical competence in nursing preferred.
• Current Georgia nursing license; BLS Provider certification required.
• Experience with chart auditing and interpreting regulations governing patient care.

This position is for a Case Manager whose responsibilities includes assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating inter-disciplinary care activities; assists with Swing Bed Program: and coordinates the continued care of transitioned patients. The position reports to the Chief Nursing Officer.

Qualifications: JOB QUALIFICATIONS

• Minimum level of Education: Education level equivalent to completion of an associate degree in nursing.
• Formal Training: Clinical competence in nursing preferred.
• Licensure, Certifications & Registration: Current license to practice professional nursing in the state of Georgia Current license to practice professional nursing in the state of Georgia; BLS Provider certification required. Instructor certifications do not replace provider certification requirements.
• Work Experience: Basic technical nursing skills. Experience in chart auditing. Experience in interpreting federal and state regulations and accreditation standards governing patient medical necessity and continuity. Basic typing and computer skills.
