Job summary

Qualifications

• Graduate from an accredited School of Professional Nursing.
• Current state licensure as a Registered Nurse (RN).
• BLS certification through AHA or Red Cross.
• Valid Ohio driver's license with required auto insurance.
• Minimum one year clinical experience as an RN.
• Minimum one year in acute medical-surgical or home care setting.

Responsibilities

• Provide high quality nursing care to patients in their homes.
• Cover the Tuscarawas County area including New Philadelphia, Dover, Sugarcreek and surroundings.
• Learn to provide assessments, complex nursing treatments and education to help avoid readmissions.
• Document patient care in medical records and collaborate with care team.
• Participate in residency core curriculum and work with experienced nurses for coaching.

As a Home Health Registered Nurse, you will be responsible for providing high quality nursing care to patients in the comfort of their own homes. This position will cover the Tuscarawas County area including the cities of New Philadelphia, Dover, Sugarcreek and other surrounding villages and townships. If short staffed, this role could require caregivers to travel to other counties.

As a Home Health Registered Nurse, you willbe responsible forprovidinghigh qualitynursing care to patients in the comfort of their own homes. An RN in this role will learn to provide assessments, complex nursing treatments, and education to help avoid readmissions to the acute care setting. Cleveland Clinic Home Care provides a generous in-depth orientation that is tailored toeach individualand their experience level. Work alongside seasoned nurses who are eager to educate and coach new graduates.

A caregiver in this position will work Monday through Friday from 8:00am to 5:00pm. On call requirements occur Friday night to Monday morning. Weekend rotations are also a requirement. Evenings occur based on patient needs.

A caregiver who excels in this role will:

• Participate in a residency core curriculum based on individual learning needs.
• Provide direct nursing care alongside asuccesscoach.
• Establish and/or revise priorities for patient care (acuity of need, patientpreference,and resource availability).
• Respond to dataindicatingrisk to patients’ health and initiate action to correct, reduce or prevent risk.
• Seeksupervision,consultation,andassistancewhen unable to perform safely and independently.
• Documentinpatients’ medical records.

Minimum qualifications for the ideal future caregiver include:

• Graduate from an accreditedSchool ofProfessionalNursing
• Current state licensure as a Registered Nurse (RN)
• Basic Life Support (BLS)CertificationthroughtheAmerican Heart Association (AHA)or the American Red Cross
• ValidDriver's License (VDL) in Ohio with proof of $100,000/$300,000 automobile insurance coverage, uponhire andper policy 8-175 thereafter
• Minimum one year current clinical experience as a registered nurse.
• Minimum one year experience in an acute medical-surgical or home care setting.

Preferred qualifications for the ideal future caregiver include:

• Bachelor's of Science inNursing (BSN)

Physical Requirements:

• Requires full range of motion, manual and finger dexterity and eye-hand coordination.
• Requires standing and walking for extensive periods of time.
• Requires corrected hearing and vision to normal range.
• Requires some exposure to communicable disease or bodily fluids.
• Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
• Physical Demand requirements are in excess of those for Light Work.
• Must be ambulatory and able to drive to patients' homes, able to climb stairs, pull, push, and facilitate movement of the patient.
• Must be able to read, write and communicate with patients.

Personal Protective Equipment:

• Follows standard precautions using personal protective equipment as required.
