Mass General Brigham is seeking an RN Case Manager (Transplant) to join our on-site team at 55 Fruit Street in Boston. You will collaborate with physicians, nurses, and support staff to optimize the patient journey across the continuum of care.

Responsibilities include assessment, planning, implementation, coordination, monitoring, advocacy, and evaluation of patient needs, with emphasis on efficient care, cost containment, and timely reimbursement; ADN or BSN required, MA RN license.

Qualifications

• RN Case Manager experience required.
• Must be a licensed RN in Massachusetts.
• Minimum of 2 years Med/Surg experience.
• Case Manager certificate preferred.
• ADN required, BSN strongly preferred.
• Acute hospital experience preferred.
