Job summary

Whittier Rehabilitation Hospital is seeking a seasoned Case Manager for the Transitional Care Unit to coordinate patient discharge planning and discharge planning across the care team.

The role requires an RN, with 2–5 years of acute care case management experience, and Massachusetts licensure. You will work within CMS guidelines to ensure safe, timely discharges and optimal rehabilitation services for our patients.

Qualifications

• RN licensure in Massachusetts is required.
• 2–5 years of acute care case management experience preferred.
• Excellent written and verbal communication, interpersonal and organizational skills.
• Knowledge of CMS guidelines and criteria for short term SNF care would be beneficial.

Responsibilities

• Oversee and coordinate each patient's TCU stay from admission through discharge.
• Collaborate with the interdisciplinary care team to identify goals and address barriers to discharge.
• Provide patients and families with information about rehabilitation processes, levels of care, and rights/responsibilities.
• Advocate for patients within CMS guidelines and Managed Care expectations when possible.
• Identify and provide resources for next level of care (e.g., SNFs, home health agencies).
• Maintain regular contact with patient and family regarding progress and discharge planning.
• Address disposition changes promptly and anticipate discharge needs.
