Job summary

Whittier Rehabilitation Hospital in Westborough, MA is seeking a part-time Case Manager to coordinate discharge planning and patient education for medically complex patients. You will collaborate with the care team to develop realistic goals and ensure smooth transitions to next levels of care.

The role emphasizes rapid initial assessment, strong communication, and effective coordination with families, payors, and community resources to optimize patient outcomes.

Qualifications

• Excellent communication (written and verbal) and strong interpersonal/organizational skills.
• Ability to assess, analyze, plan, implement and evaluate patient care and discharge needs.
• Understanding of insurance concepts, benefits, limitations, and coordinating funding for services.
• Knowledge of growth and development across the life span.
• Case management experience preferred.

Responsibilities

• Meet with patient/family within 1 working day of admission to assess discharge planning needs.
• Provide patient/family information about rehab process, levels of care, homecare services, and rights.
• Attend interdisciplinary team meetings to establish treatment plans with realistic goals and target dates.
• Collaborate with care team and family for timely discharge and identify agencies for next level of care.
• Make referrals and coordinate transfers to maintain continuity of services; obtain payor approvals when needed.
• Organize referral paperwork and ensure completeness for discharge referrals.
• Maintain patient privacy and confidentiality at all times; release information only as authorized.
• Act as utilization review coordinators and monitor patient care levels.
• Refer cases requiring medical judgment to a physician advisor.
• Maintain contact with external case managers and payors for continued stay reviews and approvals.
• Notify team and business office of exclusions or level of care changes.
• Attend UR Committee meetings as assigned and participate in improvement initiatives.
• Document all case management activities in the medical record.
