Job summary

CVS Health in Louisiana is seeking a Registered Nurse with case management experience to coordinate all case management activities and develop strategies to improve members' wellness within network policies.

The role requires active RN licensure, strong communication and motivational interviewing skills, excellent documentation, and the ability to travel up to 125 miles for 1-day fieldwork, with a 40-hour workweek.

Qualifications

• Current/Active Clinical Licensure (RN)

Responsibilities

• Coordinates all case management activities with members to evaluate medical needs and to facilitate the overall wellness of members.
• Develops strategy to address issues to outcomes and opportunities to enhance members overall wellness through integration.
• Instructs programs and procedures in compliance with network management and clinical coverage policies.
