Job summary

Vytalize Health is hiring an RN Case Manager to partner with clinical teams and physician practices, advocating for personalized treatment within a value-based care model. You will educate, navigate, and manage plans of care for patients with chronic or serious conditions, prioritizing patient-centered outcomes and cost-efficient care at home.

The role emphasizes telehealth delivery, collaboration with PCPs/med teams, and seamless coordination across the continuum of care to improve health

Qualifications

• Bachelor's degree in Nursing or equivalent RN qualification.
• Unencumbered RN license, compact license preferred.
• ACM certification preferred.

Responsibilities

• Assess and engage patients needing clinical support to determine priorities.
• Deliver patient-centered care and education within scope of practice.
• Coordinate care with PCPs, NPs, and health team to keep patients stable at home.
• Act as liaison with physician practices and community resources.
• Navigate care pathways and address barriers, including SDoH.
