Job summary

Vytalize Health seeks an RN Case Manager to partner with clinical teams and physician practices, advocating for personalized, value-based care, educating patients, and managing care plans for chronic or serious conditions. The role emphasizes telehealth, care coordination, and empowering patients at home.

Ideal candidates have RN licensure, 5 years in RN or case management, and strong communication and organizational skills. Remote work options are available as part of a fully remote team.

Qualifications

• Bachelor’s degree in Nursing preferred or ADN with relevant experience.
• 5 years’ RN or RN Case Manager experience in complex care management.
• Minimum of 3 years’ experience in Med Surg or Home Health.
• ACM certification preferred.
• Active RN license, compact license preferred.
• Excellent communication and organizational skills.
• Ability to work remotely as part of a fully remote team.
• Telehealth care management familiarity.

Responsibilities

• Engage patients needing clinical support to determine and prioritize needs; deliver patient-centered care.
• Identify education needs via telephonic assessment and support transitions from inpatient to home.
• Conduct timely telephonic clinical outreach to identified patients.
• Coordinate care with PCPs, NPs, and other clinicians to keep patients stable at home.
• Serve as the point of contact for patients, families, payers, and community resources.
• Implement interventions to improve outcomes, reduce costs, and enhance patient experience.
• Collaborate with provider offices, SNFs, hospitals, and other teams to meet patient needs.
• Maintain confidentiality while coordinating across the continuum of care.
• Guide patients through the healthcare system and overcome barriers.
• Coordinate treatment and services and schedule medical appointments.
