Job Description

Tridant Solutions is seeking an experienced licensed RN case manager who can support Air Force Service members. You will act as social workers, advocate soldier welfare, and serve as a liaison between soldiers, their families, and healthcare providers. You will lead and support a team of LVN/LPN and other case management staff.

DUTIES:

  1. Develop, implement, and evaluate individualized soldier care plans.
  2. Responsible for tracking medical status of ALL assigned Soldiers.
  3. Responsible for medical charting in appropriate databases.
  4. Request profile exams/evaluations and audiology exams at physical exam center.
  5. Coordinate Soldier treatment plans with appropriate medical facilities.
  6. Maintain automated referrals indicated in databases.
  7. Submit case processing, medical documentation, and profile concerns to military/contacted provider.
  8. Attend PHA events as determined by the needs of the state and the individual units.
  9. Ensure those Soldiers referred by a provider are followed by the case management team.
  10. Manage Soldier’s care from the identification of the condition to when the Soldier is either:
  • Considered medically ready.
  • Administratively separated from the military.
  • Must have experience with electronic medical records (EMR) (i.e., eCase, MedChart, etc).
  • Salary DOE + benefits

    Ten (10) holidays per year

    Job Type: Full-time

    Salary: $26.00 to $35.00 /hour

    Qualifications

    Must possess Case Management experience with preference in supporting US military service members. Must be computer literate to enter and maintain case management databases. Prefer experience with eCase, MedChart, IPERM & AHLTA.

    Must have RN license. Must be able to pass a background check.
