Job summary

LifeLung, Inc. (DBA: Tridant Solutions) is in search of an experienced licensed RN case manager to support Air Force Service members. Your role involves advocating for soldiers' welfare and acting as a liaison between soldiers, their families, and healthcare providers.

You will lead a team of LVN/LPN staff, develop individualized care plans, and ensure continuous medical care throughout the soldier's journey. Comprehensive understanding of electronic medical records is essential.

This full-time position offers a salary range of $26.00 to $35.00 per hour plus benefits.

Qualifications

• Must be a licensed RN.
• Experience in case management for US military service members required.
• Ability to pass a background check.

Tridant Solutions is seeking an experienced licensed RN case manager who can support Air Force Service members. You will act as a social worker, advocate soldier welfare, and serve as a liaison between soldiers, their families, and healthcare providers.

You will lead and support a team of LVN/LPN and other case management staff.

• Develop, implement, and evaluate individualized soldier care plans.
• Track medical status of all assigned Soldiers.
• Document medical charting in appropriate databases.
• Request profile examinations, evaluations, and audiology exams at physical exam center.
• Coordinate Soldier treatment plans with appropriate medical facilities.
• Maintain automated referrals indicated in databases.
• Submit case processing, medical documentation, and profile concerns to military/contacted provider.
• Attend PHA events as determined by the needs of the state and the individual units.
• Ensure Soldiers referred by a provider are followed by the case management team.
• Manage Soldier's care from identification of the condition to when the Soldier is either considered medically ready or administratively separated from the military.

Must have experience with electronic medical records (EMR) such as eCase, MedChart, IPERM, and AHLTA.

Salary DOE + benefits. Ten (10) holidays per year. Full-time.

$26.00 to $35.00 per hour.
