Job summary

Morris County Hospital in Council Grove, KS seeks an experienced Case Manager and Swing Bed Coordinator to lead discharge planning, utilization review, care coordination, and the Swing Bed program. Qualifications

• Current, unrestricted RN license in Kansas; RN strongly preferred for clinical utilization review and nursing assessment responsibilities.
• Bachelor’s degree in nursing, social work, healthcare administration, or a related field preferred.
• Experience in case management, discharge planning, utilization review, post-acute care, or Swing Bed operations preferred.
• Knowledge of medical necessity, payer processes, community resources, advance directives, and interdisciplinary care planning.
• Case-management certification preferred.

Responsibilities

• Lead discharge planning, utilization review, care coordination, and Swing Bed program.
• Collaborate with patients, families, providers, nursing, therapy, community partners, and payers to promote appropriate admissions, safe transitions, efficient throughput, and continuity of care.
• Complete discharge-planning assessments, coordinate services, facilitate family meetings, and arrange community referrals.
• Monitor barriers to care, readmission risk, and transition needs; accelerate concerns promptly.
• Maintain referral relationships with surgery centers and post-acute providers; support outreach materials.
