Job summary

U.S. Department of Veterans Affairs in Omaha, NE is seeking an RN Case Manager for Specialty Medicine Clinics. The RN supports patient education, triage, and coordination of care within a Whole Health System, under the Specialty Medicine Clinic Nurse Manager.

The role includes reviewing medical histories, medications, and coordinating consults and records while cross-training to assist other services. Typical schedule is Mon-Fri with no telework.

Qualifications

• Active RN license in the United States.
• Experience in specialty clinics and patient education.
• Strong communication and care coordination skills.
• Ability to manage referrals and clinic scheduling.

Responsibilities

• Provides patient education and telephone triage.
• Reviews patient history, medications, allergies, and other information to ensure safe care.
• Assist in patient intake, vitals, and rooming as needed.
• Coordinate care with providers; manage consults and external records requests.
• Prepare clinic by reviewing labs/imaging needs; serve as backup for other RN Case Managers.
