Job summary

Community Medical Centers is seeking an RN Case Manager to provide seamless care coordination from admission through discharge, supporting the physician and interdisciplinary team to improve outcomes and patient satisfaction while managing care costs. This role integrates utilization management, care facilitation and discharge planning within a collaborative team.

This position requires California RN licensure, a Bachelor's in Nursing, and BLS; 1 year of acute care experience is required, or 3

Qualifications

• Bachelor's Degree in Nursing is required.
• 1 year of acute care hospital experience required.
• 3 years of acute care hospital experience may be substituted in lieu of Bachelor's Degree and experience requirement.
• Current and valid Registered Nurse license to practice in California required.
• Current Basic Life Support for Healthcare Providers (BLS) from American Heart Association required.

Responsibilities

• Responsible for providing support and expertise through a continuum of care from admission through discharge.
• Supports the physician and interdisciplinary team to facilitate patient care, while enhancing clinical outcomes and patient satisfaction and managing the cost of care.
• The RN Case Manager incorporates admission status and medical necessity determination to ensure appropriate care and services are delivered efficiently and timely.
