Job summary

St. Mark's Hospital in Salt Lake City, Utah, is seeking a Case Manager (CM) to ensure high-quality, patient-centered rehabilitative care.

The CM coordinates and directs care with the Rehab Program Director and Facility Case Management Director, focusing on optimum patient outcomes within the rehab team. The CM will lead patient education, coordinate family conferences, implement treatment plans, document assessments, and advocate for exceptional patient service.

Qualifications

• RN licensed with an associate degree in nursing.
• BSN preferred.
• InterQual experience preferred.
• Certification in Case Management preferred.
• Basic Life Support (BLS) required.
• 2+ years hospital case management or 3+ years critical care nursing experience.

Responsibilities

• Provide program orientation to patients/families/caregivers.
• Coordinate patient/family/caregiver education and involvement in plans.
• Ensure implementation of treatment plans aligned with patient needs.
• Document Discharge Planning Evaluation and psychosocial assessments.
• Promote consistent, positive patient interactions and service excellence.
