Job summary

POSITION SUMMARY – RN Case Manager – Public Health

POSITION DUTIES – Public Health RN Case Manager provides comprehensive case management and public health services, including well-child care, immunizations, infectious disease screening and investigation, chronic disease management, and patient education. This role supports and trains Community Health Aides/Practices, helps develop and improve public health programs, travels to rural communities to provide services and on-site instruction, coordinates care with other providers, maintains accurate patient records, and promotes quality outcomes through evidence-based public health practices.

MINIMUM REQUIRED QUALIFICATIONS –

• AK RN license
• BLS
• Diploma, Associate of Applied Science, or Associate’s Degree in Nursing for RN
• 2 years of positive work history as RN or other public health related field. A higher-level degree in related field may be considered in lieu of equivalent professional experience.
• Knowledge and experience with Pediatrics
• Computer experience
• Must be adept with Cerner and its modules within 3 months of hire. Must be proficient with equipment and tools used in an EPSDT exam within 3 months of hire.
• Experience in teaching, working in cross-cultural setting and knowledge of the Community Health Aide Program
• Fluent in English

PREFERRED QUALIFICATIONS –

• Bachelor of Science in Nursing or Master of Science in Nursing
• ACL and PALS
• Proficiency in MS Office software including WORD, EXCEL and ACCESS
• Fluent in Yugtun

LENGTH OF ASSIGNMENT – 13 weeks

SHIFT / HOURS PER WEEK – Days (32hrs/wk guaranteed)
