Shifts Available: Monday - Friday, 8a-5p

Daily Responsibilities For The RN Case Manager

• Coordinate end-to-end case management for patients, including care plan development, specialist and diagnostic referral navigation, chronic disease support, patient education, and follow-up to improve health outcomes and ensure a seamless care experience


• Collaborate closely with an integrated primary care team to provide hands‑on clinical support (assessments, injections, phlebotomy, IV fluids, vital signs), while delivering compassionate, patient‑centered care through both in‑office and virtual interactions
