Job summary

Qualifications

• Bachelor's degree required or a nurse working on their degree with an RN license.
• 5+ years working in a hospital setting.
• Strong interpersonal and communication skills; ability to document accurately.

Responsibilities

• Completes utilization review screens for inpatient and observation cases to ensure appropriate level of care.
• Reviews short/long/complex cases to support safe discharge planning and avoid delays.
• Reviews Medicare and other payer criteria for admission and authorization.
• Coordinates with physicians, payers, and transfer centers for authorization prior to discharge or transfer.
• Documents acuity and plan of care consistently in the EMR and tracks necessary data.
• Supports discharge planning and transitional care with interdisciplinary team.
