Elevance Health seeks an OB/NICU/PEDs Nurse Case Manager I to perform telephonic care management for members with complex and chronic care needs. The role requires a BA/BS in a health-related field, a current NC RN license, and 3 years of clinical experience. Multistate licensure may be required.

The position supports a hybrid/virtual work model with in-person training sessions and needs NC residency to be within state guidelines. Prior case management certification is preferred.

Qualifications

• Requires BA/BS in a health related field and minimum of 3 years of clinical experience.
• Current, unrestricted RN license in applicable state(s) required; multi-state licensure if providing services in multiple states.

Responsibilities

• Perform care management within licensure scope for members with complex health needs.
• Develop, implement, coordinate, monitor and evaluate care plans to optimize member health care across the care continuum; duties performed telephonically.
