Job summary

Muskego Health & Rehabilitation Center is seeking an experienced RN Case Manager / Assistant Director of Nursing to coordinate resident care from admission through discharge while supporting the Director of Nursing.

The role blends resident-centered care coordination with clinical leadership, requiring strong assessment, discharge planning, and collaboration with an interdisciplinary team to maintain high-quality outcomes.

Qualifications

• Current, active Wisconsin RN license.
• Experience in long-term care, skilled nursing, rehabilitation, or related setting preferred.
• Strong clinical assessment, organizational, communication, and leadership skills.
• Knowledge of care planning, discharge planning, and regulatory requirements.
• Ability to work with residents, families, healthcare providers, and an interdisciplinary team.
• Supervisory or nursing leadership experience preferred but not required.

Responsibilities

• Coordinate and monitor resident plans of care from admission through discharge.
• Complete and monitor nursing assessments and identify changes in resident condition.
• Communicate with residents and families regarding care needs, progress, and transitions of care.
• Assist with discharge planning and coordination of services following discharge.
• Monitor changes in resident condition and ensure appropriate follow-up and interventions.
• Maintain accurate, timely, and complete clinical documentation.
• Identify individual social and emotional needs.
• Develop and maintain individualized care plans.
• Assist the Director of Nursing with daily operations of the nursing department.
• Provide clinical leadership, guidance, mentoring, and support to nursing staff.
• Participate in Quality Assurance and Performance Improvement (QAPI) activities.
• Assist with survey preparation, regulatory readiness, and follow-up activities.
