Constellation Health Services is seeking a dynamic Home Health RN to join our team in Manhattan, NY. This field-based position requires driving to treat patients in homes, assisted living facilities, and skilled nursing facilities.

Responsibilities include skilled nursing assessments, Plan of Care development, Oasis familiarity, and educating patients and caregivers. Strong communication with the Clinical Manager and timely iPad documentation are essential.

Qualifications

• RN license in the state of employment ready. Must have 1–2 years of RN experience; home care preferred.
• Oasis experience strongly desired for patient admissions, re-certifications, transfers and discharges.
• Experience with managing PleurX catheters, Foley catheters, wound vac systems, Ostomy care is a plus.

Responsibilities

• Provide skilled nursing and head-to-toe assessments for adults and geriatric patients.
• Deliver visiting home health services to an assigned caseload.
• Develop the Plan of Care with the healthcare team.
• Educate patients and caregivers on disease, meds, and treatment plans.
• Report to Clinical Manager daily and document via iPad within required timeframes.

Constellation Health is seeking a dynamic Home Health Registered Nurse to join our team in Manhattan, NY!Constellation Home Care is looking for a compassionate and skilled Registered Nurse (RN) Case Manager to join our team!
