Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to deliver compassionate, in-home hospice care and lead patient-centered care planning.

You will conduct assessments, develop and adjust plans, administer treatments, and guide families through end-of-life conversations with empathy and clinical excellence. The role requires an RN with an accredited nursing degree (Associates, Bachelors, or Masters) and at least 1 year of acute care experience; hospice/home health experience is

• RN license/degree required (associates, bachelor’s, or master’s in nursing).
• 1+ year of recent acute care experience required; hospice/home health experience preferred.

Responsibilities

• Conduct initial and ongoing home assessments to determine hospice needs.
• Develop individualized care plans with patient and family; update as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques and symptom management.
• Participate in on-call rotations and coordinate seamless care transitions.
