Job summary

SunnyBrook Home Care is seeking a full-time Registered Nurse Case Manager for the Albia area. This role focuses on one-on-one home care with clients in their homes, offering on-the-job training and support from a local management team.

NEW GRADS WELCOME; Home Health experience preferred but training provided. Flexible scheduling Monday–Friday 8:30 am–4:30 pm with on-call rotation and mileage reimbursement.

Qualifications

• Active Iowa RN Licensure.
• CPR Certification or ability to obtain.
• Must have a valid driver's license, insurance and reliable transportation.
• Prior home care experience is preferred but will train the right candidate.
• Experience with OASIS, Medicare/Medicaid regulations, and EMR (Wellsky/Kinnser) software preferred but will train the right candidate.
• Ability to work independently with good time management/organizational skills as well as part of a close supportive team.
