Job summary

Qualifications

• Active Iowa RN licensure.
• CPR certification or ability to obtain.
• Valid driver’s license, insurance and reliable transportation.
• Home health experience preferred but on-the-job training provided.
• Familiarity with OASIS, Medicare/Medicaid regulations and EMR software preferred.

Responsibilities

• Provide caseload management for home health clients in Muscatine.
• Coordinate care with clinicians, patients and families to meet care plans.
• Document care activities in the EMR per OASIS/Medicare guidelines.
• Assist with referrals, intake and care transitions as needed.
• Participate in on-call rotation and maintain quality standards.
