Job summary

The role collaborates with ED physicians, hospitalists, and the care team to develop individualized discharge plans, review utilization, and communicate across payers.

Qualifications

• Graduate of an accredited school of nursing.
• Preferred: Bachelor's degree in nursing (BSN).
• Current registered nurse license in state of practice.
• BLS from American Heart Association.
• Preferred: CCM or ACM certification.
• Experience in discharge planning, case management or utilization review is a plus.

Responsibilities

• Performs pre-admission, admission, and continued stay reviews per CMS guidelines and payer policies.
• Acts as liaison between ED physicians, hospitalists, care team, patient, caregiver, and third-party payers.
• Develops and implements discharge plans based on patient needs.
• Maintains knowledge of participation requirements and ensures compliance with regulations.
• Communicates effectively with all parties and manages discharge coordination.
• Adapts communication to diverse patient populations while respecting privacy and consent.
