Job summary

Community Medical Centers in Fresno, CA is seeking an experienced RN Case Manager to support a continuum of care from admission through discharge. You will collaborate with physicians and the interdisciplinary team to improve clinical outcomes, patient satisfaction, and cost of care.

The role requires a California RN license, a Bachelor's in Nursing, BLS certification, and typically 1 year of acute care; 3 years may substitute.

Qualifications

• Bachelor's Degree in Nursing is required.
• 1 year of acute care hospital experience required.
• 3 years of acute care hospital experience may substitute for Bachelor's Degree and experience requirement.
• Current and valid Registered Nurse (RN) license to work within the state of California required.
• Current Basic Life Support for Healthcare Providers by American Heart Association required.

Responsibilities

• Provide support and expertise through a continuum of care from admission through discharge.
• Supports the physician and interdisciplinary team to facilitate patient care, while enhancing outcomes and managing costs.
• The RN Case Manager coordinates admission status and medical necessity to ensure timely, appropriate services.
• Incorporates utilization management, care facilitation and discharge planning into daily workflow.

Responsibilities

Responsible for providing support and expertise through a continuum of care from admission through discharge. Supports the physician and interdisciplinary team to facilitate patient care, while enhancing the quality of clinical outcomes, patient satisfaction and managing the cost of care. The role integrates and coordinates utilization management, care facilitation and discharge planning functions. The RN Case Manager incorporates admission status and medical necessity determination, to ensure that appropriate care and services are delivered efficiently and timely.

Qualifications

Education:

• Bachelor's Degree in Nursing required

Experience:

• 1 year of acute care hospital experience required
• 3 years of acute care hospital experience may be substituted in lieu of Bachelor's Degree and experience requirement

Licenses and Certifications:

• Current and valid Registered Nurse (RN) license to work within the state of California required
• Current Basic Life Support for Healthcare Providers by American Heart Association required
