Job summary

CareAparent in Minnesota seeks a Registered Nurse Case Manager (RNCM) to perform in-home assessments, develop care plans, and supervise HHA and CNA staff. This role identifies and implements home health plans for admissions under a home health episode, while educating patients and families to safely participate in care.

The RNCM will conduct Starts of Care, Recertifications, and monitor conditions, coordinating with interdisciplinary teams to deliver high-quality, person-centered care in

Qualifications

• Current Minnesota RN license
• Experience with seniors and client assessments
• Excellent verbal and written communication skills
• Able to travel to client homes and support on-call weekends/holidays

Responsibilities

• Conduct initial home health OASIS assessments and Starts of Care/Recertifications
• Supervise and coordinate LPN visits, CNAs, and other care team members
• Develop and monitor plans of care based on physician orders and patient needs
• Coordinate referrals to interdisciplinary teams and ensure care plan adherence
