Zusammenfassung

CareAparent is seeking a Registered Nurse Case Manager (RNCM) in Minnesota to conduct in-home assessments, re-evaluations, and care plan development. You will educate patients and families, supervise home care aides, and ensure adherence to the care plan while supporting patients to remain at home.

Responsibilities include coordinating referrals to therapies, overseeing LPN visits, and providing clinical leadership to the care team. Travel within the local zones is required.

Qualifikationen

• Excellent customer relations and interpersonal skills.
• Excellent verbal/written communication skills.
• Ability to work on call for rotating weekends, and holidays.
• Occasional travel to client homes is required.

Aufgaben

• Conduct initial home health OASIS assessments and setup starts of care.
• Develop and monitor care plans based on physician orders and client needs.
• Supervise HHA/CNA visits and coordinate with interdisciplinary teams.
• Prepare medical documentation and visit reports using CareAparent EMR.
