CareAparent is hiring a Registered Nurse Case Manager (RNCM) to perform in-home assessments, develop and adjust care plans, and supervise LPNs and home health aides. The role requires an active Minnesota RN license, experience with seniors and client assessments, and the ability to travel to client homes.

On-call weekends and holidays may be required. A valid driver’s license and insured vehicle are necessary.

Formación

• Current Minnesota RN license is required.

Responsabilidades

• Conduct initial home health care OASIS assessments (Starts of Care, Resumptions, Recertifications).
• Develop and monitor care plans based on physician orders and initial assessments; revise as condition changes.
• Coordinate referrals to PT, OT, Speech Therapy, Falls Prevention; ensure timely implementation.
• Supervise LPN visits and ensure adherence to the plan of care.
• Prepare visit/shift reports and patient records; lead care team conferences.
• Coordinate on-site CNA/PCA/HHA visits and updates to care plans.
• Report changes in client condition to physicians and Clinical Manager; document with EMR.
