Responsibilities

• Assess patient needs and develop, review, and update comprehensive care plans.
• Coordinate care transitions, referrals, and continuity of care across settings.
• Evaluate medical necessity and monitor length of stay.
• Coordinate insurance authorizations and optimize resource use.
• Advocate for patients and educate families on treatment and self-care.
• Collaborate with physicians, therapists, social workers, and other healthcare professionals.
