LanceSoft is seeking a Nurse Case Manager (RN) in Las Vegas, NV. This full-time role sits in Case Management, offering a pay range of $40–$63/hour with a sign-on bonus up to $15,000 and a full benefits package including Nevada PERS.

The Nurse Case Manager coordinates patient care, discharge planning, utilization review, and care management, serving as a clinical resource for patients, families, and healthcare teams, in a high-acuity academic medical setting.

Qualificações

• Active Nevada RN license required.
• BLS certification through AHA required.
• Minimum 3 years of acute care nursing experience.
• Minimum 1 year of Case Management, Discharge Planning, or Utilization Review experience.

Responsabilidades

• Assess, plan, coordinate, and evaluate patient care needs.
• Develop and manage patient care and discharge plans.
• Collaborate with physicians, nurses, and multidisciplinary teams.
• Coordinate utilization review and case management activities.
• Educate patients and families regarding treatment and care plans.
• Ensure compliance with healthcare regulations and documentation standards.
• Support continuity of care and resource management.
