Job summary

Quantum Health is seeking an experienced RN Case Manager for a hybrid role at our Dublin, OH campus.

As a key member of the care team, you will advocate for members, coordinate services, and support education for patients and providers to simplify their healthcare journey.

Qualified candidates will hold an active RN license, with CCM preferred and BSN encouraged. 2+ years in clinical care required; 5+ years preferred.

Qualifications

• Active RN license in good standing with state board of licensure.
• CCM credential strongly preferred; if not held, obtain within 24 months.
• BSN degree preferred.
• Minimum 2 years of direct patient care; 5+ years preferred.
• Excellent verbal and written communication skills; comfortable with phone outreach.
• Strong administrative and technical skills; proficient with PC, Office apps, and teleconferencing.

Responsibilities

• Identify members for case management and chronic condition activities.
• Apply nursing processes to manage members and coordinate care.
• Educate members and providers to navigate healthcare complexities.
• Document all activities in appropriate databases.
• Maintain collaborative relationships with health care teams.
• Assist members with wellness activities and enhanced benefits.
• All other duties as assigned.
