Job summary

Quantum Health in Dublin, OH is seeking a Registered Nurse (RN) Case Manager to join a cross‑functional care team. You’ll advocate for members during challenging illnesses and coordinate education, services, and care plans to navigate complex health systems.

Qualifications

• Active, unrestricted RN license in good standing in the US.
• CCM certification strongly preferred or to be obtained within 24 months.
• BSN preferred; 2+ years direct patient care experience required.
• Experience in case management/coordination of programs is preferred.
• Excellent verbal and written communication with providers and members.

Responsibilities

• Identify members for case management and chronic condition activities.
• Apply nursing processes to actively manage members and services.
• Provide education and guidance to members and providers.
• Maintain high quality of care and document activities in databases.
• Collaborate with health care teams and assist with wellness activities.
