Job summary

Quantum Health in Dublin, OH, seeks a Registered Nurse Case Manager to join its cross‑functional care team with hybrid work-from-home flexibility. No weekends or holidays. You will advocate for members and guide them through complex health care needs.

You apply nursing processes to coordinate services, educate members and providers, and document all activities to maintain high-quality care per standards and regulations. CCM preferred; BSN welcomed; 2+ years of clinical experience.

Qualifications

• Active RN license in good standing with state board of licensure.
• CCM credential strongly preferred or obtainable within 24 months.
• BSN preferred and 2+ years of clinical experience required.

Responsibilities

• Identify members for case management and chronic condition activities.
• Apply nursing processes to manage members and coordinate care.
• Educate members and providers to navigate healthcare effectively.
• Document all member activities in appropriate databases.
• Maintain quality of care per standards and regulations.
