Job summary

Cambridge Caregivers, LLC in Houston, TX is seeking an experienced RN Case Manager to join our private duty in-home care team. You will conduct comprehensive patient assessments, monitor vital signs, administer medications as ordered, and develop individualized care plans to support residents at home.

You will educate families, coordinate with the Director of Nursing, and ensure documentation and orders are followed with empathy and professionalism in a fast-paced home-care setting.

Qualifications

• Must hold current Texas RN license or compact license.
• Current CPR certification (AHA) and BLS required.
• Reliable transportation and ability to work independently.

Responsibilities

• Perform comprehensive patient assessments and document findings
• Monitor and record vital signs (blood pressure, temperature, heart rate, etc.)
• Administer medications as ordered
• Provide wound care, including dressing changes and monitoring healing
• Educate patients and families on care plans, medications, and home care strategies
• Collaborate with Director of Nursing regarding changes in patient condition
• Develop, update, and follow individualized care plans
• Maintain a clean and safe environment and accurate documentation

RN Case Manager - Houston

Private Duty In-Home Care: Cambridge Caregivers is a family-owned and operated provider of non-medical caregiver services. We are known for personalized attention and our great staff. Our caregivers are W-2 employees who have completed extensive training developed by our Director of Nursing. We have 300+ Caregivers, CNA’s, LVN’s, RN’s and EMT’s on staff in Dallas, Ft. Worth and Houston.

Registered Nurse Responsibilities:

• Perform comprehensive patient assessments and document findings
• Monitor and record vital signs (blood pressure, temperature, heart rate, etc.)
• Administer medications as ordered
• Provide wound care, including dressing changes and monitoring healing
• Educate patients and families on care plans, medications, and home care strategies
• Respond to patient concerns with professionalism and empathy
• Collaborate with Director of Nursing regarding changes in patient condition
• Perform ongoing patient data collection and maintain accurate documentation
• Develop, update, and follow individualized care plans
• Ensure all orders are followed, documented, and communicated appropriately
• Assist with emergency response measures when necessary
• Maintain a clean and safe environment
• Build trust and rapport with patients and families
• Deliver exceptional patient experience through responsiveness and care
• Serve as a resource and mentor to patient care staff
• Pursue ongoing professional development
• Support and uphold the mission, values, and philosophy of Cambridge Caregivers
• Adhere to all company policies, procedures, and standards of care

Registered Nurse Requirements:

• Current RN license in the state of Texas and/or compact license
• Current CPR Certification (American Heart Association)
• Basic Life Support (BLS) Certification
• Reliable transportation
• Ability to work independently and follow clinical direction
• Compassionate, professional, and patient focused demeanor
