Job summary

Assisted Home Health & Hospice in Ventura, CA is seeking a full-time RN Case Manager for our Hospice Team. Spanish speaking is preferred but not required. You will conduct initial and ongoing patient assessments, develop care plans, administer nursing care, and support patients and families in managing their needs.

Qualifications

• Graduate of an approved school of professional nursing.
• Current, active license in the state of California Board of Nursing.
• Current CPR, health clearance, TB clearance.

Responsibilities

• Have flexible hours and working close to home (reduce your commute!).
• Complete initial and on-going comprehensive assessments of the patient's needs
• Provide service and /or treatments requiring substantial and specialized nursing skills
• Initiate appropriate preventative and rehabilitative nursing procedures
• Counsel, support and instruct the patient, family and caregiver in meeting nursing and related needs
